General Practitioners may refer patients to the Monday Fellows Head and Neck Clinic held each Monday morning at 11am. For an appointment please call the Head and Neck clinic on 02 8514 0432. The Clinic is located at:

Important Note
Please note there is also a Monday Multi-disciplinary Head and Neck Clinic held before the Fellows Head and Neck Clinic. Patients MUST be referred by one of the Institute’s consultants to this clinic following an initial evaluation in their rooms. Please do not ask for your patient to be booked into this clinic as it will cause unncessary delays. If your patient has been referred to this clinic by a consultant the clinic is located at :

How to Refer a Patient

To refer a patient, GPs need to send us a referral letter along with the patient’s relevant medical imaging and/or test results.

Address the referral to:

Fax the form and test results
Send the completed form or referral letter by fax to (02) 9383 1033 or email to headandneck@lh.org.au and include any relevant test results showing anomalies.

Once this is received, the referral will be triaged and our Head and Neck Patient Navigator will contact the patient to arrange an appointment. If you have any questions about the referral process or the clinic you can contact us on (02) 8514 0432.


Private appointments

GPs may also refer patients to a SHNCI surgeon in their private consulting rooms. If the problem is urgent, a direct telephone call from the referring doctor to the specialist may expedite the referral process. Patients are able to choose their clinician, and require a referral to this clinician. A list of specialists can be found here. Billing occurs at the time of consultation and Medicare rebates can be claimed on site. Private patients can choose their specialist and time of treatment, will not see a registrar and will receive other in-room private patient benefits including cable television.
